Dr Desire has been a well respected practicing physician for 20+ years. His years of medical experience in caring for patients throughout the health care continuum has uniquely qualified him to be your Geriatric Physician Consultant. He has provided medical care to geriatric patients in medical offices, private homes, independent and assisted livings, adult day care, hospitals, nursing homes, and hospice.
As a Certified Medical Director (CMD), a member of the American Geriatric Society, and a member of the American Board of Post- Acute and Long- Term Care Medicine, he is able to stay on the cutting edge of the medical and social issues that effect the geriatric community.
MEDICAL CONSULTATION PLAN
Please remember, we are not your primary care doctor. We are another medical professional who is giving you up to date medical advice to enable you to make better decisions about your health with the help and guidance of your primary care doctor.
Monthly Medical Consulting Plan:
- 40 minutes a month with health care professionals via phone or video chat ( no carrying over of unused minutes to the next month)
- Monthly minutes may be shared with another geriatric patient living in the same household
- Unlimited correspondence via email (drdesire@gpcconsultants.com) - used for matters that can be addressed within 24 hours ( for any urgent matters, please call your primary care doctor or call 911 for any emergency)
- Limited texting
- Questions answered by a medical professional (not by a friend or neighbor) who knows your medical history
- Patient portal access to upload your medical records via HIPPA compliant EMR
- Any required prescriptions has to be done by your primary care doctor.
Our Services
$ 198 per patient for the initial onboarding of a patient that involves a 40 to 60 minutes session to obtain your medical information
$ 98 a month for each patient or each household ( monthly automatic debit)
a 12 months minimum commitment required for this service
